It is important that the employer fills out the blanks: name, type of work, hours employed, employer signature, company name, company address, zip code.

3. When the work permit application is completed by the employer, the student or parent presents it to the secretary in Student Services. The parent must sign the form in front of the secretary that he/she approves of the hours the student will be employed.
