
Home Hospital Procedures
East Peoria Community High School
- The Special Services office is informed that a student will not be able to
attend school for two consecutive weeks or more and the parent or guardian
requests Home/Hospital tutoring.
- The parent/guardian is provided with the Home/Hospital form and is
directed to obtain a licensed physician's certification of the illness and
anticipated length of school absence. Home tutoring will not begin until the
physician's certification is received and Hospital tutoring will not be paid
for unless the physician's certification is received and Hospital tutoring
will not be paid for unless the physician's certification is on file.
- If a student receives Hospital services for less than two weeks the
district will not pay for the services.
- The Administrative Secretary to the Director of Special Services will
locate tutor(s) for the student. At first, the student's teachers at EPCHS
will be asked if they wish to tutor. If not, other qualified persons will be
employed.
- Tutoring will be provided up to five hours per week; one hour for each
subject tutored.
- There are certain subjects that can not be offered through
Home/Hospital tutoring because of lab work or activities that must be
participated in at school
Additional Information
- Services usually begin within a few days of receipt of the physician's
recommendation. Services end when the student is released from the hospital
or the physician's recommendation terminates. The recommendation must be
renewed every 6 weeks.
- Home/Hospital tutoring is generally limited to the standard school term
and does not include summer school.
- There must be an adult present in the home when a tutor is providing
instruction.
- If, in the opinion of the Director of Special Services, the home is not a
suitable place for tutoring to occur an alternate site will be arranged.
- All assignments and grades are given by the student's teacher at East
Peoria Community High School, not the tutor. The tutor provides instruction
and serves as an intermediary.
- Students may be placed on Homebound instruction by board action, IEP
recommendation or administration recommendation as well as physician's
recommendations.
- The Student Services department is responsible for handling all
assignments distributed between students and tutors and to make sure that
students receive proper credit for their participation in Homebound
instruction.
- If a student is not attending school, but is working in the community --
this will terminate tutoring services.
